Confidentiality of patient information is very important at Women's College Hospital. Information is only released as authorized by the patient, legal representative or as required by law.

If you would like a copy of your Women's College Hospital health record, please go to the Department of Health Information (E134) to complete a request form. Health records requests will be processed within 30 days

Please note that there is no charge to view your health record or to send a copy to your health-care provider, but if you would like a copy of the records for personal use you will be charged a production fee.
