The Cardiac Rehab Program

WCHI cardiac rehab program for women

Women who are living with a heart condition or who are at moderate to high risk for heart disease can join this program designed to help manage known risk factors by the following services:

  • cardiovascular assessment by an advance practice nurse
  • individual cardiac risk factor profile
  • exercise consultation
  • heart clinic education session
  • access to the cardiac team for one-on-one counseling and support for lifestyle modification

Women who would like an assessment of their personal risk factor profile, complete a risk screening questionnaire. A meeting then takes place with a member of our team to review the assessment and plan any necessary risk factor changes. Depending on the results of the assessment, a prevention and education plan or cardiac rehabilitation will be prescribed.
