The Complex Care Clinic (CCC) was created to help you in the management of your chronic health conditions. Our goal is to stabilize and optimize chronic medical conditions and support you to improve your health. Finding your way around the health-care system when you have complex medical needs can be challenging and confusing.
At the Complex Care Clinic you will work with a comprehensive team of health-care professionals who will co-ordinate and provide the complete care you need. Timely communication between care providers is important to optimize patient care. The clinic will provide an updated care plan to you, your primary care physician and your community care service providers (if applicable).
You will be seen by a team of health professionals which, depending on your condition, may include:
This clinic offers assessment, treatment and follow-up as needed. The team will also help to co-ordinate with other clinical programs.
Given the teaching and inter-professional nature of the clinic, please come prepared for an extended appointment that may be at least two hours in duration. An inter-professional care plan is jointly developed by you and the team to assist in achieving your health goals.
