The virtual ward is a new model of care located at Women's College Hospital that is designed to support patients in conjunction with their family doctors for several weeks after hospital discharge. This is a collaborative  initiatived with Toronto Central Community Care Access Centre, St. Michael's Hospital, and the University Health Network.

Note: Referral to the Virtual Ward is through direct admission at the time of hospital discharge, from one of the above listed acute care hospitals.
