The Virtual Ward is a new model of care at Women's College Hospital, designed to support patients who are at high risk of readmission to hospital and their family doctors for a few weeks after hospital discharge. The virtual ward takes the best elements of hospital care (hence "Ward") and applies them to patients living in their own homes. (hence "Virtual")

This is a collaborative initiatived with Toronto Central Community Care Access Centre, St. Michael's Hospital, and the University Health Network. Referral to the Virtual Ward is through direct admission at the time of hospital discharge, from one of the acute care hospitals mentioned.
