Complex Care Clinic (CCC)

The Complex Care Clinic (CCC) was created to help you in the management of your chronic health conditions. Our goal is to stabilize and optimize chronic medical conditions and support you to improve your health. Finding your way around the health-care system when you have complex medical needs can be challenging and confusing.

At the Complex Care Clinic you will work with a comprehensive team of health-care professionals who will co-ordinate and provide the complete care you need. Timely communication between care providers is important to optimize patient care. The clinic will provide an updated care plan to you, your primary care physician and your community care service providers (if applicable).

Services provided
You will be seen by a team of health professionals which, depending on your condition, may include:

  • general internal medicine physician
  • registered nurse
  • pharmacist
  • dietitian
  • social worker
  • occupational therapist
  • physiotherapist
  • respiratory therapist
  • Community Care Access Centre case co-ordinator

This clinic offers assessment, treatment and follow-up as needed. The team will also help to co-ordinate with other clinical programs.

Given the teaching and inter-professional nature of the clinic, please come prepared for an extended appointment that may be at least two hours in duration. An inter-professional care plan is jointly developed by you and the team to assist in achieving your health goals.

Based on your condition, services may include:

  • a comprehensive medical assessment, further investigations and planning for disease management
  • a physical assessment, which may include a falls risk assessment
  • an assessment of psychosocial concerns that may impact your health outcomes
  • education about disease process and information on services available to provide management strategies
  • an intensive medication assessment to determine the current requirement for the medication, correct administration, and potential drug interactions
  • comprehensive health education and self management support can be provided to you and/or your caregiver.


A referral from a physician is required. Referrals can be faxed to 416-323-7739.
