Email Instructions

1. In the email you receive, click the “Register Email Consent” link.


2. Your web browser will open a WCH myHealthRecord “Verify Your Identity” web page. In the “Date of Birth” field, enter your date of birth in the format DD/MM/YY, then click “Verify”.


3. You may be asked to agree to the myHealthRecord Terms and Conditions. Review the terms and conditions, then click “Accept” to proceed or “Decline” to exit.

Note 1: This does not automatically create a myHealthRecord account for you. If you would like one, you can do so in step 8 after signing the consent form.

Note 2: Clicking “Decline” on the Terms and Conditions will return you to the main myHealthRecord log-in screen. If you click “Decline” by accident, return to step 1 above.


4. On the “My Documents” screen that appears, locate the “Email Consent Form” and then click the “Review and sign” button.


5. Review the consent form. Scroll down and choose either “Yes” or “No” for Medical Care-Related Communications and Patient Experience Feedback Opportunities.


6. Click “Sign Here” to enter your digital signature, then click “Accept” to confirm your signature.


7. Click “Accept” again to accept the form. Your email consent preferences are now submitted to WCH.


8. If you would like to sign up for myHealthRecord, WCH’s patient portal that provides you access to information about up-coming appointments, test results, and more, click “Create account” and follow the prompts.
