The Atrial Fibrillation (AFib) Clinic is an innovative, transition clinic that provides an optimal, guideline-based, individualized care for patients with Atrial Fibrillation.
We provide:
- Timely access from the ED or Primary Care Provider to the first clinic visit
- Education about AFIB and its management, tailored to the needs of the patient
- Facilitation of access to necessary cardiac investigations
- Comprehensive management of AFib as well as other comorbidities that impact AFib
- Timely access to cardiology, electrophysiology or specialty care if required
- Seamless transition to the community care provider via clear, one-page guideline-based care plans within 24-48h of the visit

The AFib team includes an interdisciplinary care team led by a Nurse Practitioner and Pharmacist, supported by a physician and fostered by partnerships to specialty care.

- Laboratory investigations
- Non-invasive cardiac testing: Stress test, Echo, Nuclear Cardiac Testing, Holter Monitor, PMIBI
- Pulmonary Function Tests and Respirology consultation
- Cardiac Rehabilitation
Referral Inclusion
- Patient must be medically stable
- Patients must be able to come for outpatient follow up.
Referral Exclusions
- Patient is medically unstable
- Patient has congenital heart disease
