COVID-19 ASSESSMENT CENTRE TESTING ELIGIBILITY
The Ministry of Health and Ontario Health have announced changes to who is eligible for a COVID-19 test at Assessment Centres. To have a COVID-19 test at our Assessment Centre, you must:
Be showing COVID-19 symptoms; or
Have been exposed to a confirmed case of the virus, as informed by their public health unit or exposure notification through the COVID-19 app; or
Be a resident or worker in a setting that has a COVID-19 outbreak, as identified and informed by their local public health unit; or
Are eligible for testing as part of a targeted testing initiative directed by the Ministry of Health or the Ministry of Long-Term Care.
To register for a COVID-19 assessment, please complete our online registration form
> Please only submit one form for each person needing a test.
> A clinician from Women’s College Hospital will call you to discuss COVID-19 testing.
> It may take up to 24 hours to receive a call after you register. These are outgoing calls only, so please leave your phone on to receive the call.
> Please note children under the age of 2 cannot be tested at this location.
