COVID-19 Self-Screening Assessment

We are asking everyone who is coming to the hospital to complete the following COVID-19 Self-Screening Assessment:

  • Do you have one or more of the following COVID-19 symptoms: a fever/chills, cough, new onset of shortness of breath, joint/muscle aches, loss of taste and/or smell, runny nose?
Covid symptoms include: cough, fever and/or chills, shortness of breath, runny nose, muscle aches and/or joint pain, loss of taste and smell
  • Have you tested positive for COVID-19 or been instructed to self-isolate by Public Health in the past 10 days?
  • Have you been in close contact with someone who tested positive for COVID-19 or who had a new respiratory illness in the past 10 days?

If you answer ‘yes’ to any of the questions, please do not come into the hospital without contacting your clinic first. Your care team will take extra steps to make sure you can safely receive the care that you need.

Please note that masks continue to be required at Women’s College Hospital. Everyone who comes into the hospital will be provided with a medical grade mask to wear throughout their visit. This includes, but is not limited to, in parking areas, elevators and waiting areas.
