Book a COVID-19 Vaccine Appointment

Before you book your COVID-19 vaccination appointment, please read the below information carefully to ensure that you are eligible. Once you have reviewed the information, click the following link to book your appointment:

Must be 12+ years of age on day of vaccination.

Pfizer monovalent for the primary series and Moderna bivalent for booster doses.

Welcome to anyone with or without health card (OHIP).
Please bring proof of age (e.g. passport, birth certificate, etc.)

Boosters are available at our clinic for those eligible.

You will need proof of age, such as a passport or birth certificate. You do not need an Ontario Health Insurance Plan (OHIP) health card. Please wear a loose fitting top or a t-shirt so the healthcare provider can easily access your arm for the vaccination.

Yes, each of the vaccines approved in Canada are safe. All vaccines are heavily inspected in clinical trials and by Health Canada. The approval process is very strong. Even after approval, Health Canada and the Public Health Agency of Canada continue to monitor the safety and effectiveness of vaccines.

We are currently only offering vaccines to those who are ages 12 and up. To find nearby clinics offering vaccines to children ages 5-11, please visit the City of Toronto website.
